Please select your service and a provider. After you select your preferred Date/Time, a new screen will open for you to login.

If you are a current patient with MIH, please use the email address we have on file here at the clinic.

All current patients WILL need to click the Forgot Password button and reset the password.

All NEW patients to MIH, please click the "create new account" option.
